Introduction

Democratic governance has not been a driving force in global health. Many of the countries that have had the greatest improvements in life expectancy and infant mortality over the last 15 years are election campaigns that have achieved their health success with the huge contribution of foreign aid. Ethiopia, Myanmar, Rwanda and Uganda have all extended their life expectancy by 10 years or more between 1996 and 2016.
